In this book I explore some of the implications of looking at healthcare ethics in social context. My starting point is the contention that clinical ethics cannot be insulated from questions in public-health ethics; and the book is in large part an elaboration and defence of that contention. My purpose is partly substantive and partly methodological—to ask what happens to ‘personal claims’ when they are set against ‘public claims’; and to see interpersonal healthcare relationships in the context of wider social relations. I hope that what I have to say will help to reframe some of the problems of healthcare ethics, and that where I do not attempt solutions this reframing can be seen as a contribution of its own.

In placing healthcare ethics in social context I am operating with two rather different but complementary senses of ‘the social’. First, I am thinking about health as a ‘social good’. I am interested in the interactions between, and what I will suggest is, in some respects, a growing convergence between personal healthcare and public health. This includes the various senses in which individual health needs to be understood in the context of population health, and the ways in which professional ethics in healthcare can only be understood in conjunction with ‘institutional ethics’ and ‘policy ethics’. This provides the substantive theme of the book.
